Aging at Home is a goal for most of us. A recent webinar sponsored by Seniors for Social Action Ontario, “Population-Based Integrated Care: Implications for Supporting Aging at Home and in the Community” reviewed Ontario’s move from LHINs (Local Health Integration Networks) to Ontario Health Teams (OHTs). OHTs are local networks of healthcare providers and organizations—including hospitals, primary care, home care, and mental health services—that work together as a single coordinated team. The 58 OHTs are organized by area. West Region includes Cambridge North Dumfries and Kitchener, Waterloo, Wellington, Wilmot & Woolwich (K4W), the municipalities of our current members.
